Capstone Health Center is always accepting new patients. We provide all levels of primary & preventative health care services.

Capstone Health accepts many major health insurance plans including Medicaid, Medicare, BCBS, UHC, TriCare, VIVA Health. We also file with other private carriers as a service to our patients. This includes all services provided at Capstone Health. Medicare patients’ deductible is waived on office visits and other approved FQHC services.

Payment is expected at time of service. Cash, check and credit/debit cards are acceptable types of payment. Payment plans or credit may be arranged for those unable to pay their entire bill.

Depending on household income and family size, patients may qualify for REDUCED charges under our sliding fee discount program. Ask the Patient Services team for more information.

No Surprise Billing Act Notice

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost for services. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of their bill for services if requested or the patient’s cost will be at least $400.
