Job Description

Capstone Health seeks a Certified Family Nurse Practitioner to join our team. The ideal candidate is service orientated, and patient driven, enjoys working with underserved patients and is culturally sensitive. Current licensure in the State of Alabama and Board Certification/Eligibility required. Primary duties will consist of treating patients in the clinic and telehealth visits. These visits may be split equally within the work schedule.

Job Duties

• Prepares patient’s chart prior to clinical exam.
• Prepares patient for clinical exam/visit, including the collection of typical patient vital metrics (blood pressure, temperature, height, weight etc.).
• Assists provider with clinical visit.
• Follows through with post-visit duties.
• Provides service in a manner that is appropriate for the patient’s age; demonstrates knowledge and skills necessary to meet the patient’s physical, psychosocial, educational, and safety needs.
• Mentors and assists students performing clinical rotations.
• Other duties as assigned.

Job Duties

• Maintain After Hours Call log and report as requested
• Participate on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the UDS, Patient-Centered Medical Home and Quality Assurance committee.
• Maintain information needed for insurance wellness programs based on which quality measures are required per insurance company
• Serve as Liaison for insurance companies for linkage to quality care and maintain appropriate documentation.
• Maintain Hospital Follow Up report and report to Quality Committee as requested
• Maintain Emergency Department follow up report and report to Quality Committee as requested
• Maintain and document incoming Medical Records pertaining to all Quality Measures
• Maintain Outgoing Medical Records and related documentation
• Promote clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
• Other duties as assigned.
