Sliding Fee Discount Program

As a Federally Qualified Health Center, Capstone Health Center provides a Sliding Fee Discount Program for all patients that are uninsured or underinsured and whose total household gross income falls at our below 200% of the federal poverty guidelines. The sliding fee discount is used to calculate the basic discount. It is updated each year using the federal poverty guidelines. Learn more about the federal poverty guidelines.


Patients with third party insurances that do not cover or only partially cover fees for certain Capstone services may also be eligible for the sliding fee discount. Eligibility is in accordance with laws and regulations of the third-party payor contracts.

How to Qualify Our Discount Program

Patients must provide updated information for the Sliding Fee Discount Program annually. Patients without insurance will need to provide the following information:


  1. Family Size
    Number of all family members living in the household.

  2. Proof of Income
    Most Current Paystub
    OR
    Tax Return, Pension, Retirement, Social Security Income, Disability Income, Child Support, Alimony, Workers Compensation Payments, Unemployment Compensation, Supplemental Security Income, Public Assistance, Veteran’s Payments, Survivors Benefits, Dividends, Rents, Royalties, Income from Estates, Trusts, Educational Assistance, and any assistance from outside the household or other miscellaneous sources.
    Proof of Income is for all family members living in the household.


  3. Valid Photo ID
    Driver License, Voter Registration Card, Student ID, Non-Drive State ID with a Photo
    Walker County Homeless Coalition (
    205-221-4010) will help get a Photo ID if for Medical purposes.

  4. Nominal Fee/Payment
    Medical Services: $20.00 (Level A). Cost then increases based on household income.

    Counseling Services: $15.00 (Level A). Cost then increases based on household income.
